Module 16 — HSPI-24 — High Sensitivity, or Autistic Sensory Difference?

 

What sensory-processing sensitivity actually is, how far it overlaps with autistic difference and where the two part company, why the popular twenty per cent figure does not hold, and the one finding in this literature that is genuinely good news.

The big idea

 

If you found the phrase "highly sensitive person" years before anyone said the word autistic, and it was the first description that ever fitted — this module is not here to take it away from you. It is here to say what the research behind that phrase actually shows, where it overlaps with autistic sensory difference, and where the two come apart.

 

Read that again. The same trait that makes a bad environment worse for you makes a good one work better. If you have spent years hearing that you are fragile, the evidence says something closer to the opposite: you are responsive. The cost of that is real, and so is the return.

 

Step 1 — The lesson

 

A. The word that arrived first

 

For a great many people reading this, the sequence went the same way. Thirty years of being told you were too much or too thin-skinned. Then a book, or a quiz, or a friend who had read the book. Then a phrase that finally described something true: highly sensitive. For some people, autistic arrives years later. For others it never arrives, and does not need to.

 

The construct underneath the phrase is called sensory-processing sensitivity. Elaine Aron and Arthur Aron proposed it in 1997 across seven studies, working up from thirty-nine qualitative interviews to a 27-item questionnaire tested in student and community samples. Internal consistency came out at .85 to .87. The scale correlated with introversion at .14 to .45 and with emotionality at .46 to .65 — high enough to be related, low enough that the authors argued they had found a third thing rather than a rewording of the first two.1

 

What most people met was the popular version: a temperament, roughly a fifth of the population, deep feeling and easy overwhelm. Aron later summarized it with the acronym DOES — depth of processing, overstimulation, emotional reactivity and empathy, sensing the subtle.2 The four dimensions the HSPI-24 gives you back are that model, with the names lightly changed.

Hold both of those at once, because the rest of this module depends on it. Sensitivity is a real, heritable, well-studied dimension of human difference. The instruments that measure it are the weakest part of the field, and the one you just took is weaker still.

 

B. Four dimensions, one number

 

The HSPI-24 gives you a total between 24 and 120 and four subscale readings between 6 and 30. Those four are worth reading one at a time, because they describe genuinely different experiences.

 

Depth of Processing is the part where a two-minute conversation gets replayed for three days. You do not make decisions quickly because you are running more of the possibilities. Overstimulation is the supermarket at five o'clock, the open-plan floor, the restaurant where everyone is shouting over the music. Emotional Reactivity is crying at the advert, or absorbing a colleague's bad morning so completely that it becomes yours. Sensitivity to Subtleties is noticing that somebody moved the chair, that the tone in a text was fractionally off, that the tap is dripping two rooms away.

The best-known published measure in this area has never managed the same clarity. In 2006 an analysis of 851 undergraduates found that the 27-item HSP Scale does not hold together as one thing at all. Three components came out: Ease of Excitation (12 items), Aesthetic Sensitivity (7 items) and Low Sensory Threshold (6 items), with internal consistencies of .81, .72 and .78. The three did not behave alike. Ease of Excitation correlated with neuroticism at .48, Low Sensory Threshold at .31, and Aesthetic Sensitivity at only .19.3 One part of "sensitivity" looks a lot like distress. Another looks a lot like an eye for beauty. Adding them together loses the distinction.

 

By 2019 the field had largely moved on from arguing about the questionnaire and started calling the underlying idea environmental sensitivity: individual differences in how strongly your surroundings register, in both directions.4 That reframing matters more than it sounds, and section D comes back to it.

 

A recent attempt to rebuild the measure from scratch put the criticism bluntly. The original scale overlaps with at least four of the Big Five traits, roughly eighteen of its twenty-seven items are negatively worded, and it fails to capture two of the four characteristics the model is built on — depth of processing and emotional reactivity.2 That is worth knowing if the version of the trait you were handed came from that scale.

 

C. Highly sensitive, or something else?

 

Here is the question the title asks, and the honest answer is not a clean one. The two constructs overlap substantially. They are also not the same, and quite a lot of people are highly sensitive and not autistic.

 

The shared ground is sensory. Overload in busy rooms, a nervous system that registers the fridge and the strip light, the need for a long quiet gap after anything social, noticing detail other people walk past. That territory belongs to both descriptions, and Module 15 maps it in more detail than this one does.

 

The size of the overlap has been measured. A twin study of 2,944 adolescents found that environmental sensitivity correlated with autistic traits at .30, with a genetic correlation of .42; about two thirds of the observed overlap was accounted for by shared genetic influences.5 That is a real relationship. It is also nowhere near identity. A correlation of .30 leaves most of the variance in each trait unexplained by the other.

Notice what a sensitivity score does not ask about. It does not ask whether you script conversations in advance. It does not ask about a lifelong difference visible in childhood, about needing things to stay the same, about interests that go far deeper than the people around you expect, about what it costs you to hold eye contact, or about the exhaustion of masking. None of that is in the questionnaire. So a high sensitivity score cannot rule autism in, and a low one cannot rule it out.

 

It is not specific in the other direction either. In 274 adults, ADHD traits correlated highly significantly with sensory sensitivity, and a factor analysis produced a factor mixing ADHD items and HSP Scale items together.6 A study of 837 people that mapped the scale into the space of eleven other traits found its components pulling apart: Ease of Excitation and Low Sensory Threshold sat beside neuroticism, Aesthetic Sensitivity beside openness. Sensitivity, the authors concluded, is distinct but not unitary.7

 

None of that makes highly sensitive a wrong answer. It is a decent description of something real, it has helped a lot of people stop apologizing, and for some readers it will remain the most accurate word available. The question worth asking is narrower: is it doing work it cannot actually do? If the label has been carrying your social exhaustion, your childhood, your need for routine and your scripts as well as your sensory life, it is carrying more than the evidence puts in it.

 

D. What the number cannot settle

 

Start with the instrument in front of you. The HSPI-24 was built in-house by New Path, and its own page says it is in development and has not been validated through formal scientific research. That is not a footnote to be softened. It means there is no published validation, no normative sample, no reliability figure and no evidence about what any score predicts.

 

It also means the band boundaries are drawn rather than derived. The line at 59 and the line at 79 were not produced by comparing groups or by any statistical procedure on a sample. They are reasonable places to divide a range. They are not thresholds, and moving from 78 to 81 is not an event.

 

The published literature is in better shape, and it still does not support the popular claim. When 906 adults were sorted by latent class analysis, three groups came out, not two: 30.5 per cent low, 40.3 per cent medium, 29.2 per cent high. The authors' own conclusion was that environmental sensitivity is a continuous and normally distributed trait on which people fall into groups, rather than a category you belong to or do not.8 The tidy idea that a fixed fifth of the population is highly sensitive does not survive contact with the data.

Then there is the question of whether sensitivity is its own thing. A twin study of adolescents estimated the heritability of environmental sensitivity at 47 per cent. Of that genetic influence, roughly 80 per cent was shared with personality traits — principally neuroticism and extraversion — leaving about 20 per cent specific to sensitivity itself.9 So the trait is real and inherited, and most of what makes it heritable is not unique to it.

 

One more finding should change how you read your own total. Across 1,628 adults, the highly sensitive did not form one group. Two came out with opposite personality profiles: one with above-average neuroticism and introversion, the other with average neuroticism and above-average extraversion and openness.10 A high score does not tell you which of those you are, and those are two very different lives.

 

Now the part of this literature that is genuinely good news, and genuinely well supported. Sensitivity was originally studied as a vulnerability — more sensitive, more harmed by bad environments. The evidence turned out to be two-sided. The framework is called vantage sensitivity: the same people who suffer more in poor conditions gain more from good ones.11 In a school-based depression prevention program for at-risk eleven-year-old girls in England, with 166 in the treatment group and 197 controls followed for twelve months, the program reduced depression in the girls scoring high on sensitivity and did nothing at all in the girls scoring low.12 In 395 Japanese adolescents, the highly sensitive students started with lower wellbeing and lower self-esteem than their peers, and then responded more strongly to a resilience program, with a greater reduction in depression and a greater gain in self-esteem.13

 

INSIDE THE INSTRUMENT

 

HSPI-24 — Highly Sensitive Person Inventory, New Path's own instrument

 

Where it came from

 

The HSPI-24 was written in-house by New Path and its live page states that it is in development and has not been validated through formal scientific research. It is not an adaptation of any licensed instrument. Its four dimensions — Depth of Processing, Overstimulation, Emotional Reactivity and Sensitivity to Subtleties — correspond to the DOES framework Elaine Aron uses to summarize sensory-processing sensitivity, the construct she and Arthur Aron introduced across seven studies in 1997.1 The construct has an evidence base. This questionnaire does not.

 

What it is made of

 

Twenty-four items, four subscales of six items each, a total range of 24 to 120 and subscale ranges of 6 to 30. Total bands are set at 24 to 59, 60 to 79 and 80 to 120; every subscale uses the same three bands at 6 to 12, 13 to 17 and 18 to 30. The live page does not state the response scale used for individual items. Scores are raw, not standardised, and not comparable to any published cut-off on any published instrument.

 

How well it performs

 

Unknown. No internal consistency, no test-retest reliability, no factor analysis, no convergent or discriminant validity and no sensitivity or specificity figures have been published for this questionnaire. For context on what a validated measure in this area looks like: the original 27-item HSP Scale reported alphas of .85 to .87 across its development samples,1 and a later four-factor instrument built on the DOES model reported ordinal alpha and omega of .71 to .85 and thirty-day test-retest reliability of .77 to .89.2 The HSPI-24 has no equivalent figures.

 

Where it was validated

 

Nowhere. There is no normative sample, no clinical sample and no published validation study. The band boundaries at 59 and 79 were not derived empirically. The parent literature's own norms are also thinner than the popular account implies: the largest latent class analysis found three sensitivity groups at 30.5, 40.3 and 29.2 per cent of 906 adults, with HSP Scale cut-points of 3.71 and 4.66, while concluding the underlying trait is continuous and normally distributed.8

 

What it cannot do

 

It cannot separate sensitivity from autism, and no sensitivity measure can. Environmental sensitivity and autistic traits correlated at .30 phenotypically and .42 genetically in 2,944 adolescent twins, with roughly two thirds of the phenotypic overlap explained by shared genetic influence.5 Substantial overlap, well short of identity. Nothing on this or any sensitivity questionnaire assesses social communication, restricted or repetitive behavior, insistence on sameness, developmental history or masking.

 

It is not specific to any condition. ADHD traits correlated highly significantly with sensory sensitivity in 274 adults, and factor analysis produced a mixed ADHD-and-HSPS factor.6 Elevated sensitivity scores are common across anxiety and mood presentations, and the sensory territory itself is transdiagnostic, as Module 15 sets out at greater length.

 

The construct is not cleanly separable from personality. A twin study put the heritability of environmental sensitivity at 47 per cent, of which about 80 per cent was shared with personality traits, chiefly neuroticism and extraversion, leaving roughly 20 per cent specific.9 In a trait-space analysis of 837 people, Ease of Excitation and Low Sensory Threshold sat beside neuroticism while Aesthetic Sensitivity sat beside openness, leading the authors to describe sensitivity as distinct but not unitary.7

 

The high scorers are not one group. Across 1,628 adults, two qualitatively different high-sensitivity profiles emerged — one with above-average neuroticism and introversion, the other with average neuroticism and above-average extraversion and openness.10 A total score does not distinguish them, which is a direct argument against treating any number here as a clinical indicator.

 

The measurement problem is inherited, not local. The most widely used scale in this literature has a disputed factor structure — three components in an 851-participant analysis rather than the intended one, with neuroticism correlations ranging from .19 to .48 across them3 — overlaps at least four Big Five dimensions, is roughly two thirds negatively worded, and omits two of the four DOES characteristics.2 A screener built on that model inherits every one of those problems before adding its own.

 

What a clinician does with it

 

Treat the four dimensions as an interview schedule and ignore the total. The clinically useful material is the profile and the history behind it: which dimension is loud, in which settings, with what functional cost, and what the person has already built to manage it. Where a client arrives holding a highly sensitive person identity, it is usually worth exploring rather than correcting — the useful question is whether the label has been carrying explanatory weight for social, developmental or repetitive features it was never designed to hold. Frame any feedback around environmental sensitivity rather than fragility, because the vantage-sensitivity evidence supports it: high-sensitivity participants responded to a preventive intervention that produced no effect at all in low-sensitivity participants.12

 

Validity tier: 4 — built in-house by New Path. Honest and useful, not normed, no published validation. The underlying construct has thirty years of peer-reviewed work behind it and a contested measurement history; this particular questionnaire has neither validation nor norms, and no number it returns should be treated as though it did.

STRENGTHS LENS

 

You are not fragile. You are responsive, which is a different thing entirely.

 

The version of sensitivity most people were handed is a deficit story: easily overwhelmed, needs protecting, cannot cope with what others manage. The research does not read that way. It reads as amplification. The same wiring that makes an open-plan office cost you two hours of recovery is the wiring that makes good supervision, a well-designed room, a decent relationship or a well-built program land harder on you than on the person next to you. That is a return, and it is measurable.

 

There is a second thing worth naming. In interviews with twelve adults scoring high on sensitivity and high on wellbeing, what distinguished them was not less sensitivity. It was self-awareness, self-compassion, solitude balanced against close relationships, contemplative practice, time in nature and work that meant something — with difficulty saying no showing up as the main barrier.14 None of that is a personality transplant. It is a set of things you can build, and some of it you have already built without being given credit for it.

 

F. What helps

 

This module has spent a lot of words on what a sensitivity score cannot settle. Here is what you can do with one anyway.

 

1. Read the four dimensions and ignore the total.

 

Write down your four subscale scores and rank them. Depth of Processing at the top is a different life from Overstimulation at the top: the first calls for time and lower decision loads, the second calls for changes to rooms and schedules. The total adds four unlike things together and tells you their sum, which is the least informative number the screener produces.

 

2. Test the label against what it does not cover.

 

List the things about you that highly sensitive has been explaining. Then cross off everything genuinely sensory. What is left — scripting conversations, needing plans not to change, interests that go far past what people expect, exhaustion after socialising that was pleasant — is outside the construct entirely. If the remainder is substantial, that is the question worth taking further, and the earlier modules in this course are where to take it.

 

3. Go and get a good environment, deliberately.

 

The vantage-sensitivity evidence says your return on a better setting is larger than average, which makes environment a high-yield place to spend effort rather than an indulgence. Pick one thing: the room you work in, who you spend Saturday with, whether there is anything green in your week. Change it properly and give it a month. You are the person most likely to notice the difference.

 

4. Keep overload and anxiety in separate columns.

 

The part of the sensitivity construct that correlates hardest with neuroticism is ease of excitation, which means the questionnaire cannot tell your overload from your worry. You often can. For a fortnight, write down each bad moment and mark it O or A: too much input, or a thought about the future. Overload is treated by changing the room. Anxiety is not, and treating one as the other wastes years.

 

5. Keep the word if it works, and keep it accurate.

 

Highly sensitive is a useful description and there is no reason to give it up, particularly if it was the first thing that ever made sense of you. It is a trait on a continuum, not a diagnosis and not a category with a wall around it. Holding it that way leaves room for it to be the whole answer, part of the answer, or the door you came through on the way to a different one.

A raised score is also not specific to autism, occurring alongside ADHD traits, anxiety and low mood, so no result here indicates that you are or are not autistic. Only a clinician can diagnose.

Ease of Excitation and Low Sensory Threshold clustered with negative traits and bordered neuroticism, while five of the seven Aesthetic Sensitivity items clustered with positive traits next to openness; the authors concluded sensitivity is a distinct but non-unitary trait rather than an artefact of neuroticism. A 16-item version with the Aesthetic Sensitivity items removed reached alpha .92 and remained separable from neuroticism. The finding directly undermines the assumption that a high total score identifies one kind of person. Limitation: cross-sectional self-report in German-speaking online samples, and latent profile solutions are sensitive to the indicator set chosen, so the number of profiles is not a fixed property of the trait.

Sensory Processing Sensitivity in the context of Environmental Sensitivity: a critical review and development of research agenda. Neuroscience and Biobehavioral Reviews, 98, 287-305. DOI 10.1016/j.neubiorev.2019.01.009. View the paper The field's principal review, written by a multidisciplinary group including the construct's original authors and its main critics. It relocates sensory-processing sensitivity inside the broader framework of environmental sensitivity, examines whether the trait is continuous or categorical, reviews measurement approaches, temperament and personality overlap, and links to typical and atypical development. Its central conclusion is two-sided: sensitivity raises the risk of stress-related problems in negative environments and also produces greater benefit from positive and supportive ones. Limitation: a narrative critical review rather than a meta-analysis, with no pooled effect sizes, and several authors have a direct stake in the construct they are reviewing.

 

2. Gubler DA, Janelt T, Roth M, Schlegel K, Guggisberg J, Troche SJ (2024). The DOES Scale: measuring sensory processing sensitivity as a trait constellation. Journal of Personality Assessment, 107(2), 205-220. DOI 10.1080/00223891.2024.2405536. View the paper Three studies (N = 471 German-speaking; N = 242 German plus N = 232 English; N = 420 including 169 self-identified highly sensitive people) developing a scale around Aron's four DOES characteristics: Depth of Processing, Overstimulation, Emotional Reactivity and Empathy, and Sensing the Subtle. A four-factor correlated model fitted better than a second-order model in every sample, with ordinal alpha and omega of .71 to .85 and thirty-day test-retest reliability of .77 to .89. The authors document the original HSP Scale's problems directly: an inconsistent factor structure, overlap with at least four Big Five traits, roughly 18 of 27 items negatively worded, and failure to capture two of the four DOES characteristics. Limitation: a new instrument validated by its own developers in convenience samples that were largely German-speaking and self-selected, with no clinical or diagnostic criterion applied.

 

13. Kibe C, Suzuki M, Hirano M, Boniwell I (2020). Sensory processing sensitivity and culturally modified resilience education: differential susceptibility in Japanese adolescents. PLOS ONE, 15(9), e0239002. DOI 10.1371/journal.pone.0239002. View the paper 395 Japanese high school students aged 15 to 16 (174 male, 221 female) in Tokyo completed a culturally adapted resilience education programme. Highly sensitive students began with significantly lower wellbeing, lower self-esteem and lower self-efficacy and higher depression than their peers, and then responded more strongly to the intervention, showing a greater reduction in depression and a greater gain in self-esteem. The authors frame this as hidden efficacy consistent with differential susceptibility: an average programme effect can conceal a large effect confined to sensitive participants. Limitation: a single-country school sample with no long-term follow-up reported, and self-report outcome measures throughout.

 

8. Lionetti F, Aron A, Aron EN, Burns GL, Jagiellowicz J, Pluess M (2018). Dandelions, tulips and orchids: evidence for the existence of low-sensitive, medium-sensitive and high-sensitive individuals. Translational Psychiatry, 8, 24. DOI 10.1038/s41398-017-0090-6. View the paper Latent class analysis of Highly Sensitive Person Scale responses in 906 adults, split into two subsamples and replicated in independent samples of 417 and 503 participants. Three classes emerged rather than the two implied by the popular account: 30.52 per cent low-sensitive, 40.29 per cent medium-sensitive and 29.19 per cent high-sensitive, with scale cut-points of 3.71 and 4.66. The authors' stated conclusion is that environmental sensitivity is a continuous and normally distributed trait on which people nonetheless fall into three groups along the continuum. Limitation: latent class analysis will return classes from continuous data by construction, the samples were largely convenience-recruited adults, and the cut-points are sample-specific rather than established norms.

 

6. Panagiotidi M, Overton P, Stafford T (2020). The relationship between sensory processing sensitivity and attention deficit hyperactivity disorder traits: a spectrum approach. Psychiatry Research, 293, 113477. DOI 10.1016/j.psychres.2020.113477. View the paper Study of 274 non-clinical adults recruited from a university volunteers list, using the Highly Sensitive Person Scale and the Adult ADHD Self-Report Scale. Self-reported ADHD traits correlated highly significantly with sensory sensitivity, and exploratory factor analysis produced a factor combining ADHD items with HSP Scale items, with ADHD traits and age both predicting sensitivity scores. This is direct evidence that a raised sensitivity score is not specific to autistic traits. Limitation: a non-clinical convenience sample with no diagnosed participants and no diagnostic interview, so it establishes trait overlap rather than any clinical relationship.

 

11. Pluess M, Belsky J (2013). Vantage sensitivity: individual differences in response to positive experiences. Psychological Bulletin, 139(4), 901-916. DOI 10.1037/a0030196. View the paper The paper that names and sets out vantage sensitivity: the proposal that individual differences in response to positive experiences are a phenomenon in their own right, not merely the mirror image of vulnerability to adversity. It reviews behavioural, endophenotypic and genotypic markers that predict who benefits most from supportive environments and interventions, and distinguishes vantage sensitivity from resilience and from diathesis-stress models. Limitation: a theoretical review rather than an empirical study, with no pooled effect sizes, and much of the supporting evidence at the time came from post-hoc moderation analyses that are prone to false positives.

 

12. Pluess M, Boniwell I (2015). Sensory-Processing Sensitivity predicts treatment response to a school-based depression prevention program: evidence of Vantage Sensitivity. Personality and Individual Differences, 82, 40-45. DOI 10.1016/j.paid.2015.03.011. View the paper Two-cohort treatment and control study of at-risk eleven-year-old girls in England, with 166 in the treatment group and 197 controls, followed for twelve months after a school-based depression prevention programme. Sensory-processing sensitivity significantly moderated response: the programme reduced depression scores in girls scoring high on sensitivity and was not effective at all in girls scoring low. This is the clearest single demonstration that a moderator can turn a null average effect into a real one for a subgroup. Limitation: a non-randomised two-cohort design in girls only, at a single developmental stage, with a moderation finding that has not been replicated in an adult sample.

 

3. Smolewska KA, McCabe SB, Woody EZ (2006). A psychometric evaluation of the Highly Sensitive Person Scale: the components of sensory-processing sensitivity and their relation to the BIS/BAS and Big Five. Personality and Individual Differences, 40(6), 1269-1279. DOI 10.1016/j.paid.2005.09.022. View the paper Factor analysis of the 27-item HSP Scale in 851 Canadian undergraduates (257 men, 594 women, mean age 19.7). Contrary to the scale's intended unidimensionality, three factors emerged: Ease of Excitation (12 items, alpha .81), Aesthetic Sensitivity (7 items, alpha .72) and Low Sensory Threshold (6 items, alpha .78), intercorrelated at only .23 to .45. The components related very differently to personality: neuroticism correlated .48 with Ease of Excitation, .31 with Low Sensory Threshold and .19 with Aesthetic Sensitivity, while Aesthetic Sensitivity's strongest association was with openness at .37. Limitation: a single undergraduate sample from one university, cross-sectional and entirely self-report, so it establishes that the scale is multidimensional without settling what the correct structure is.

14. Black BA, Kern ML (2020). A qualitative exploration of individual differences in wellbeing for highly sensitive individuals. Humanities and Social Sciences Communications, 6, 103. DOI 10.1057/s41599-020-0482-8. View the source Interpretative phenomenological analysis of semi-structured interviews (25 to 110 minutes, mean 49) with 12 Australian adults aged 19 to 69 who scored high on both sensory-processing sensitivity and wellbeing, drawn from a larger quantitative sample of 430. Three themes emerged: a conception of wellbeing centred on low-arousal states such as calm rather than high-arousal happiness; enablers including self-compassion, self-awareness, solitude balanced with close relationships, contemplative practice, nature and meaningful work; and barriers including physical health difficulties and trouble saying no. Participants described adapting themselves to an outgoing cultural norm and questioning why their energy was less consistent than other people's. Limitation: 12 participants, 11 of them women, all Australian and all selected for high wellbeing, so the account describes what thriving looks like in this group rather than what is typical of highly sensitive people generally.
